1. About These Terms
These Terms apply to gmshealthplans.com, a website operated by Golden Milestone Services, an independent insurance agency owned by Tai Thao, a licensed insurance agent. By using this site or contacting us through it, you agree to these Terms. If you do not agree, please do not use the site.
2. What This Site Is
This site provides general information about Health Insurance Marketplace (ACA) coverage and lets you request help from a licensed agent. It is a starting point for a conversation — not an enrollment system and not a quoting engine.
3. No Cost to You
We do not charge you a separate fee for our enrollment assistance. As an independent agency, we may be compensated by an insurance carrier when someone enrolls in a plan through us.
4. We Do Not Offer Every Plan
We do not offer every plan available in your area. Plan availability, pricing and eligibility for financial help vary by county, household size and income. For the complete list of options in your state, visit HealthCare.gov or call 1-800-318-2596 (TTY 1-855-889-4325).
5. Information on This Site Is General
Everything on this site is general information about how coverage works. It is not medical, legal or tax advice, and it is not a determination of what you qualify for. Rules, plan designs, premiums and subsidy amounts change. Only a completed application through the Marketplace or a carrier determines your actual eligibility, price and coverage.
Nothing on this site is an offer of insurance or a guarantee that you will be approved for or eligible for any plan or subsidy.
6. Information You Give Us
When you request help, you agree to give us information that is accurate and complete to the best of your knowledge. Quotes, subsidy estimates and eligibility depend entirely on what you tell us. Inaccurate information can lead to the wrong plan, an incorrect subsidy, or a repayment obligation at tax time.
You confirm that you are the person whose information you are submitting, or that you are authorized to submit it on that person's behalf.
7. Contacting You
When you submit a request or book a call, you are asking us to get back to you, and you agree that we may contact you at the phone number and email address you provide in order to answer that request. How we handle your information, and how to stop hearing from us, is described in our Privacy Policy.
8. Third-Party Services and Links
This site links to and relies on services we do not control, including HealthCare.gov, insurance carrier websites, our appointment scheduling provider and our form provider. We are not responsible for the content, availability or practices of those services.
9. Our Content
The text, design, graphics and other material on this site belong to Golden Milestone Services unless otherwise noted. You may read, print and share it for your own personal use. Please do not republish it commercially or present it as your own.
10. Limitation of Liability
This site is provided "as is." To the fullest extent permitted by law, Golden Milestone Services is not liable for indirect, incidental or consequential damages arising from your use of this site or reliance on general information found here. Nothing in these Terms limits any liability that cannot be limited by law, including our obligations as a licensed insurance agency.

These Terms are governed by the laws of the State of Wisconsin, without regard to its conflict-of-law rules.
